Patch Request

Please keep the following in mind:

  • You need to have a valid Trusted Person in the intranet associated with the Client ID
  • Only complete forms can be taken in as an order
  • If you are unsure about the technical details, please contact us before filling in the form
  • Delivery time for a regular order is 4-5 days
  • Delivery time for a rapid response order is 1-2 days
  • GYRO Center/Sentia can only guarantee this delivery time after receiving a complete and correct form including LOA (if needed)
  • Rates in this form are exclusive of VAT


1 Authorizer details

Patch Requests can only be done by authorized people. To confirm this, you need to use an e-mail address that has been registered as Trusted Person for the given Client ID. This Trusted Person also needs to have the 'Patch Request' permission ticked.

You can find this in the intranet at the left top side. The given Trusted Person must be associated with this client.

Please use international format without leading zero's or +, in The Netherlands starting with 316

3 Patch details

Patches will be delivered with a keystone in the patchpanel in your cabinet. Therefore you don't need to provide us with connector details at your side. To connect a patch, you either need an LC/PC fiberpatch or UTP with RJ45 connector. We have these in stock if needed.

Rapid Response delivery guarantee (+ € 50,- one-time)

At your request we can also connect the patch to your equipment. You can give exact details below.
